"To whom it may concern. I have already requested this in the past. Would you please make a note and not send me any more of these things to fill in. My eyesight has declined, my eyes are very sore when I'm on the computer and still have to use a magnifying glass. I have used Clinicuans for a long tine and will continue to do so while I can. Im always gratfull. but dint want any extra paper work to deal with. I already have to get help and things wwill have to change. No more of these please. Beryl Crisp"
